We want to understand you, your pain and your history, so you will be expected to fill out paperwork when you arrive if you did not complete online forms in advance. There will be questions about your health habits and medical history. We want to cover all bases so that we can help you as much as possible. Be sure to tell us about all the pain you are experiencing and any other symptoms you may be having.
Dr. Apps will discuss your condition and treatment options with you. Together, you can come up with a plan for treatment.
Payment Options:
Payment is expected at the time of service; we take cash, checks, credit and debit cards. We welcome your insurance including Medicare and Medicaid/Forward and will do everything that we can to help you receive as many benefits from it as possible.
People often wonder if their insurance will cover chiropractic treatment. Since there are so many different plans and coverage options available, we are not able to give you a definite answer in advance. When you arrive at your appointment, we will make a copy of your insurance card so that we can estimate what your insurance will cover.
